How It Works

LOG IN TO FUTURE
Log in to your FUTURE Health account and click "Get a Clinic Appointment”.

BOOK AN APPOINTMENT
Select the appointment you need, choose a doctor or clinic from the list, and follow the instructions to book an appointment.

GO TO THE DOCTOR
Arrive early for your appointment and pay with your FUTURE Health card. That is it!

How many office visits can I schedule?
Schedule as many office visits as you need. All visits are $99.
Where can I find a doctor?
It is easy to find a doctor or clinic. Log in to your account, click "Get a Clinic Appointment”, select the appointment you need, and choose a doctor or clinic from the list.
Are FUTURE Health clinics available in every state?
FUTURE Health partner clinics are in 48 states, including DC. FUTURE Health is not offered in Hawaii or Alaska.
What if I need an x-ray or imaging?
Imaging services like x-rays offered through our partner clinics are included in the $99 office visit fee. FUTURE Health does not provide imaging services through hospitals or non-partner clinics. Contact your FUTURE Health advocate to get discounts for imaging services provided at hospitals and free-standing imaging centers.
What about emergency room visits and surgery?
FUTURE Health does not provide emergency care or surgery. If you are experiencing an emergency, please call 911 immediately.
Can I get a referral from my primary care doc?
Yes. Your primary care provider can refer you to another doctor or specialist if medically appropriate.
What about specialty care?
FUTURE Health offers $99 office visits for primary care services only, including visits to an OBGYN for preventative and reproductive primary care. It does not include other specialty services.
Can I use FUTURE Health while traveling?
Yes. You can see a doctor at any of our 20,000+ partner clinics and urgent care facilities across the country. FUTURE Health is not available in Hawaii and Alaska.
